Job Description
Community Liaison is responsible for managing and growing assigned sales territory through the generation of home health patient referrals. The role involves daily travel by car throughout the sales territory visiting accounts. Will be tasked with identifying and prioritizing opportunities for the branch.
Will lead a collective marketing effort to meet branch and territory-specific goals.

Job Responsibilities:
Market to physicians, hospitals, skilled nursing facilities, senior living, discharge planners, and case managers to promote our services.

Develop referral relationships through a combination of scheduled and unscheduled account visits.

Meet with patients when appropriate to ensure a smooth transition to clinical services.

Provide education about service capabilities and industry-related topics.

Assists branch operational team in the collection of patient documentation from referring accounts/facilities.
Positive winning attitude

Home Health sales or operational experience is preferred.

Hospice, DME, or other healthcare sales experience calling on discharge planners, case managers, and social workers are preferred.

Familiarity with the post-acute environment is required.

Excellent interpersonal communication and presentation skills are required.
