PURPOSE AND SCOPE: Supports the mission, core values and customer service of Kidney Care Center (KCC).
Adheres to the Kidney Care Center Policies and Procedures and Compliance Program including all state and federal regulatory requirements.
The CKD (Chronic Kidney Disease) Patient Tracking Specialist is responsible for tracking all KCC patients classified with Chronic Kidney disease focusing on patients stage 3b-5, ensuring these patients have a current appointment, maintain up-to-date data for each CKD patient on the tracker and produce reports with accuracy.
PRIMARY DUTIES / ACTIVITIES:
· Identify patients that are classified with Chronic Kidney Disease stage 3b-5
· Compiles and maintains current and accurate data for all CKD patients
· Generate Excel reports identifying the CKD patients using the EHR system, Athena Health
· Arrange raw and gathered data to conform to report(s) required specifications
· Responsible for generating and maintaining monthly reports: CKD tracker GFR < 40, CKD tracker GFR < 20, CKD patient count, and CKD no appointment
· Call and schedule CKD tracker patients to ensure they have up to date appointments
· Identify patients for removal and follow up with providers regarding these patients
· Distribute monthly reports to appropriate parties
· Audit Reports to ensure accuracy
· Other duties, as assigned
CUSTOMER SERVICE:
· Responsible for supporting the values and customer service standards of KCC.
· Accountable for outstanding customer service to all staff, clients and vendors
· Develops and maintains effective relationships through effective and timely communication.
· Initiates action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.

EDUCATION:
· 3 years’ experience as Medical Assistant
· Nephrology experience preferred.
EXPERIENCE AND REQUIRED SKILLS:
· Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft Office Suite: Outlook, Word and Excel
· Good presentation & communication skills
· Must be productive & resourceful with the ability to meet deadlines
OTHER QUALIFICATIONS
· Must be a self-starter who can work independently yet function in a highly collaborative, fast-paced team environment
· Ability to multi-task, work under pressure and meet deadlines effectively
· Excellent written and oral communication skills
· Strong data entry, organizational and interpersonal skills
· High integrity, team player with patient service focus and flexibility
